Introduction

This Telehealth Consent explains how remote clinical services ("telehealth" or "telemedicine") work on the ForHumanity Health platform ("we," "us," "our"). By signing, clicking "I agree," or otherwise opting in, you (the "patient") acknowledge that you have read, understood, and agree to every section below.

Who Can Use Our Service

Our telehealth service is intended for individuals 18 years or older (or the legal age in your jurisdiction). Do not use the service if you are under 18 or if local laws prohibit telehealth in your location.

Telehealth Consent

Telehealth uses secure electronic communications so licensed ForHumanity Health providers—physicians, nurse practitioners, registered nurses, medical assistants, and other authorized clinicians—may evaluate, diagnose, advise, and treat you from a different location. You may invite family members, caregivers, or legal representatives to join a session and share your information with them at your discretion.

Information Transmitted via Telehealth

During a telehealth session, the following types of data may be transmitted through internet or telecommunications channels:

- Progress reports, assessments, and other clinical documents  

- Video, audio, images, text messages, and other digital data  

Privacy, Protected Health Information (PHI), and HIPAA

Federal and state privacy laws, including HIPAA, safeguard your health information. By using our service, you consent to the collection, use, storage, and sharing of your protected health information with necessary third parties as outlined in the **ForHumanity Health Privacy Policy**. Identifiable data will not be disclosed without your permission except for treatment, payment, healthcare operations, or as required by law.

Patient Responsibilities & Communication

Important messages from your provider will appear in your secure ForHumanity account. Check your account regularly; failing to do so may delay your care. For non-urgent questions, message your provider through the platform. Responses may take up to one business day depending on when the message is sent.

Technology Limitations

Telehealth sessions depend on internet connectivity. Signal disruptions (poor video/audio quality, dropped connections) can occur and may hinder interaction. You release and hold harmless ForHumanity Health and your care team for any data loss or delays due to technical failures.

Limited Medical Record Access

The health information you provide during a telehealth visit may be the only data available to your provider at that time. Provide complete and accurate information to ensure appropriate care.

Withdrawal of Consent

You may withhold or withdraw consent to telehealth at any time and opt for in-person care instead. Withdrawing consent will not affect future benefits or services for which you are eligible.

Release and Hold Harmless

You agree to release ForHumanity Health, Inc. and its providers from liability for technical failures, loss of data, or incomplete medical records inherent to telehealth interactions.
